We recognise the importance of consistency and we endeavour to ensure the same health care worker supports you at each visit. All our support staff are carefully selected, experienced and committed to providing quality care and support. We can provide you with care on the days that you choose and at the times which work best for you.
We offer a range of support services to meet your individualised needs, lifestyle and requirements. We can provide services in the comfort of your own home and can support you to access your community as and when you require.
We offer the opportunity to spend pre-arranged dedicated time per week, with one of our compassionate companion care workers. Time may be spent having social companionship in your home, or we can accompany you to a social group, event or appointment. We can also provide support to explore a new group or activity or to continue to attend an existing one.
2-24 Hour Care
We provide flexible support services to meet your needs and requirements. We can support you from a minimum of 2 hours up to 24 hours depending on your needs, lifestyle and situation. Our 24 hour in home care support service enables our client’s to continue to reside in the comfort of their home, with peace of mind for both the client and their families, that their individualised needs are being met and independence is maximised, ensuring their safety.
We can provide overnight care in your home on a short or long term basis. This can be scheduled to enable care partners the opportunity to take a break from their caring role during the night, or alternatively to provide additional support for client’s living at home independently, to ensure their safety. Whatever the circumstances, we can provide peace of mind that your needs are being met by one of our compassionate health care workers.
We offer client’s and their care partners short term options to have a scheduled break. This may be from a few hours to few days, depending on your situation. It can be scheduled to occur over holiday periods or while you are recovering from illness or injury. Respite is offered in the comfort of your own home and is structured around your routines and lifestyle.
We recognise the impact dementia can have on an individual and their family members and understand that each person’s needs are different and their journey with dementia is unique. We offer a range of home based and community support services for client’s living with dementia, to meet their changing needs and lifestyle and can provide respite options for care partners and families to allow regular scheduled breaks and time out.
We offer community and socialisation services to enable client’s to continue to access their community and maintain important social connections. We can provide support with shopping tasks, errands, medical appointments and/or assistance to engage in new or existing recreational or social groups and activities.
We recognise the impact that long term health conditions can have on individuals and their families and understand that it can feel overwhelming trying to access and organise different health services. We can support you to access the right support, at the right time, to help you achieve integrated care that enhances health and well-being. Our Care Navigators work alongside client’s and their families, taking a person-centred approach to help facilitate, coordinate and link client’s to the services that best fits their individualised needs.
